Here's something we see constantly: a company in aviation, finance, or healthcare gets excited about AI, runs a proof of concept, gets promising results, and then the project dies in compliance review. Not because the technology doesn't work — because nobody thought about governance until the end.

The industry conversation around AI is overwhelmingly about models, benchmarks, and capabilities. That's fine if you're a tech startup. If you're a regulated firm, the model is maybe 20% of the problem. The other 80% is everything that surrounds it: who can access what, how decisions are traced, where data lives, and how you prove all of this to an auditor.

Audit Trails: The Non-Negotiable Foundation

Every AI interaction in a regulated environment needs to be traceable. Not just 'we log inputs and outputs' — fully traceable. That means capturing the model version, the prompt template, the retrieval context (if using RAG), the user who initiated the request, and the timestamp. All of it, immutably stored.

In aviation, if an AI system assists with maintenance scheduling or safety documentation, you need to demonstrate to your national aviation authority exactly what information the system used to produce its output. In finance, MiFID II and similar frameworks require that automated decision-support systems maintain records that can be reconstructed years after the fact.

We implement this as a structured logging pipeline that runs parallel to the AI inference chain. Every request gets a correlation ID. Every piece of retrieved context gets logged with its source document and version. The logs go to append-only storage — not your application database, not a log file that gets rotated. Proper immutable storage with retention policies that match your regulatory requirements.

Explainability: What It Actually Means in Practice

Explainability is one of those words that gets thrown around without anyone agreeing on what it means. In a regulatory context, it has a specific, practical meaning: can you explain to a non-technical auditor why the system produced a particular output?

For generative AI, this is harder than for traditional ML. You can't point to feature importance weights the way you can with a gradient-boosted tree. What you can do is build systems that make their reasoning transparent by design. That means citation-grounded outputs — every claim the AI makes should reference the source document it drew from. It means confidence scoring so users know when the system is uncertain. And it means human-in-the-loop checkpoints for high-stakes decisions.

We've found that the most effective approach is to treat the AI as a research assistant, not a decision-maker. It surfaces information, cites its sources, and flags its confidence level. A human makes the final call. This isn't a technical limitation — it's a governance architecture that regulators understand and accept.

Data Residency and Access Control

Data residency sounds simple until you start implementing it. Your data needs to stay in a specific jurisdiction? Fine. But does that include the embeddings generated from that data? The model weights if you fine-tune on that data? The logs of AI interactions that contain fragments of the original data? The answer to all of these is usually yes, and most architectures don't account for it.

We architect AI systems with data sovereignty as a first-class constraint, not an afterthought. That means embedding generation happens within the required jurisdiction. Vector databases are region-locked. API calls are routed through region-specific endpoints. Fine-tuned model weights are treated as derived data subject to the same residency requirements as the training data.

Access control is equally critical and equally under-designed. Most AI deployments we audit have a single API key shared across teams. That's unacceptable in a regulated environment. We implement role-based access control at the prompt level — different teams see different data, can access different document collections, and have different escalation thresholds. An analyst in one business unit shouldn't be able to query documents from another unit's regulatory filings just because they both use the same AI tool.

Where Projects Actually Stall

After working on AI governance across aviation and financial services, we've identified the three points where projects consistently get stuck:

The compliance team sees the AI for the first time at the deployment gate. If your compliance and legal teams aren't involved from the architecture phase, you're going to redesign significant portions of your system. Involve them early. Give them a governance framework to react to, not a finished product to reject.

Nobody owns the AI risk register. AI introduces novel risks — hallucination, data leakage, prompt injection, model drift. These need to live in a formal risk register with assigned owners, mitigation strategies, and review cycles. We've seen organizations where the AI team thinks compliance owns this, and compliance thinks the AI team owns this. Nobody owns it.

The vendor contract doesn't cover AI-specific obligations. If you're using a cloud AI provider, your existing cloud contract almost certainly doesn't address model training on your data, data retention in inference logs, or liability for AI-generated outputs. These need to be negotiated explicitly.

Building Governance That Doesn't Kill Velocity

The goal isn't to make AI adoption slow and painful. It's to build a governance framework once that lets you deploy AI use cases quickly and repeatedly. Think of it as the compliance equivalent of a CI/CD pipeline — upfront investment that pays off in speed later.
